1. Student Application (Form Below) Select appropriate form.
2. Helpful Information About Your Child (Form Below) Select appropriate form.
3. Health Care Summary (Form Below)
4. Child Care Immunization (Obtain from Dr)
5. Copy of Birth Certificate or Passport
6. Transcripts from previous school (if applies)
7. Copies of Any Evaluations (Psych, Speech, Neuro etc)
8. Copies of Any Education Plans
