enrollment process.
Please fill out one form per child enrolled.
A non-refundable registration fee is required upon
submission of application.
Printed Forms can also be obtained from Hampton Academy.
The following is considered to be a complete Application
Package:
1. Student Registration Form (Form Below)
2. Helpful Information About Your Child (Form Below)
3. Health Care Summary (Form Below)
4. Immunization Card (copy)
5. Birth Certificate or Passport (copy)
6. Transcript or last Report Card from previous school
7. Copies of Any Evaluations (Psych, Speech, Neuro, Medical etc)
8. Copies of Any Education Plans or Goals
